Project description

The overall objective of this joint project is the development, testing and (process) evaluation of a prevention and aftercare management system using sensor technology for the contactless recording of health data, a multimodal, telemetric interaction system and AI-supported predictive analytics to avoid post-clinical complications and revolving door effects using the example of patients with chronic heart failure and post-Covid syndrome-related heart disease. With the help of the hybrid, multimodal interaction system, patient-oriented communication between patients, their caring relatives, family doctors and clinics as well as the transfer and integration of health data between these stakeholder groups is to be made possible.
